Dog Owner Agreement and Liability Waiver

NOTE: We require all vaccinations to be administered by a veterinarian and require a copy of the vaccination records, listing either the date administered or the expiration date. Dogs with vaccines updated after their expiration are subject to a 7-14 day waiting period, depending on the type of vaccine and how it is given. This Owner Agreement and Liability Waiver (“Agreement”) is entered into between Canine Social Club, LLC (“CSC”) and the undersigned owner of the dog(s) identified below. In order for CSC to accept my dog(s) into their facility I agree to the following terms and conditions:

GENERAL RULES

• I agree my dog(s) will be on a secure leash when arriving and departing CSC.

• I agree to schedule a specific time for drop off and pick up of my dog(s). If I do not pick up my dog(s) as scheduled, additional charges may be billed for the additional time my dog(s) remains at CSC.

• If my dog(s) is not picked up within five days of their scheduled pick up, I agree to surrender and release all ownership rights to said dog(s) and agree CSC may take lawful steps with regards to my pet(s), regarding rehoming or surrender. All reasonable measures to reach myself and my emergency contacts will be made by CSC.

• ​My dog(s) has/have current Bordetella, Rabies and Distemper vaccinations. These vaccinations were administered by a licensed veterinarian or within the parameters of the CSC Vaccination Policy.

• I will provide CSC a copy of my dog’s current vaccination records. I understand a waiver can be secured for the distemper vaccination requirement, provided my veterinarian provides written communication stating my dog(s) should not receive the vaccine. I understand waivers cannot be secured for Bordetella and rabies vaccines.

• I understand vaccinations must be updated prior to their expiration date to avoid a waiting period between administration and my dog(s)’s arrival at CSC.

• I agree if any insects or pests, including but not limited to, fleas or ticks are found on my dog(s) while at CSC, I will pay the cost of any necessary bathing my dog(s) and additional kennel cleaning to ensure others are not exposed to the pests. CSC will contact me in the event pests are found.

• My dog(s) has not been sick in the last 14 days

• My dog(s) does not have open wounds or stitches.

• I agree to provide all required medication/ointments/supplements my dog(s) may need while at CSC. Each time my dog(s) arrives at CSC, I will provide clear concise instructions for administering said products and confirm with a signature the information CSC has on file is correct. I will bring prescription medication and supplements in its original container. 

• I agree my dog(s) will not be allowed to leave CSC until all charges due are paid in full or both parties agree upon payment arrangements. If collection proceedings are necessary, I will be responsible for any and all attorney fees, regardless of whether an action is filed.

• I understand and agree prepaid packages are nonrefundable and expire 6 months from purchase date. Discounted packages are provided only if they are paid for in advance.

• I agree that CSC is not responsible for the loss or damage of any of my or my dog(s)’s personal property, including but not limited to: lost or damaged leashes, collars, beds, toys, or other items.

• I have disclosed all known behavioral issues, including aggression, fear reactions, history of bites, resource guarding, or destructive tendencies, to CSC.

RELEASE AND WAIVER OF LIABILITY

• I will not hold CSC responsible for any and all illnesses contracted by my dog(s), including any and all expenses related to any and all illnesses my dog(s) may contract.

• I have read the information provided to me on the CSC website and available in paper form in regard to CIRDC (Canine Cough). I understand some canine diseases are contagious and I assume the risk associated with my dog(s) coming into close contact with other dogs while at CSC.

• To the fullest extent permitted by Iowa law, I agree to release, waive, discharge, indemnify, and hold harmless CSC, its owners, employees, contractors, and agents (“Released Parties”) from any and all claims, liabilities, damages, losses, injuries, causes of action, or expenses arising out of or relating to my interactions with CSC, including but not limited to the following: Services provided by CSC; My dog(s)’s presence, behavior, or actions while at CSC; Any injury, harm, damage, or illness to my dog(s), myself, or to any third party; Loss, theft, death, illness, or escape of my dog(s); Damage caused by my dog(s) to any property or person.

• I understand this release applies to and is binding upon my heirs, family members and anyone else asserting such a claim.

VETERINARY CARE AUTHORIZATION.

• If my dog(s) becomes ill or injured, the CSC may, in its discretion, seek emergency veterinary care, provided I am fully responsible for all resulting veterinary bills, medications, treatments, or transportation costs. In the event of this type of emergency, I will be notified by CSC as soon as possible.

• If my dog(s) becomes ill or injured, CSC has my permission to consult my regular veterinarian or an emergency veterinarian and provide such care or treatment as may be recommended. I agree the expense for such consultation or necessary treatment will be paid by me.

• I understand, while highly unlikely, it is possible for elderly dogs to pass away while at CSC. If this should happen, I will be contacted immediately. If I cannot be reached, CSC will call the emergency contact listed on my account. If neither party can be reached, CSC will contact my dog’s veterinarian. I understand immediate transport of my dog(s) would be required to my veterinarian or the BluePearl Dog Hospital. I further understand costs and fees associated with this transport will apply and will be my sole responsibility.

PHOTOGRAPHY RELEASE

• I hereby grant CSC permission to photograph or record my dog(s) and use such images and my dog(s)’s name for social media, training documentation, or marketing purposes without compensation.

GROUP/PACK PLAY

• I understand all dogs are involved in group/pack play while at CSC.

• I understand and agree Iowa law requires dogs 6 months of age and older to be spayed or neutered in order to play with other dogs. At CSC, unaltered dogs under 6 months of age are allowed to play with other dogs.

• I understand and acknowledge that participation in daycare, group play, off-leash activities, and boarding involves inherent risks, including but not limited to: Injuries from dog-to-dog or dog-to-human interaction (scratches, bites, abrasions, ligament injuries, etc.); Stress-related conditions; Transmission of parasites, viruses, or communicable diseases; and Escape, running off, or unpredictable animal behavior.

• I expressly accept and assume all such risks as outlined above.

By signing below, I understand and approve the above terms and authorize CSC to provide services for my dog(s). I understand that this is a legally binding contract intended to limit the CSC’s liability to the fullest extent permitted by law.
